Revolve problem

Hi

I were building a simple cap model in Moi now and came across this weird meshing type issue, i'm not sure why but it seem to happen every time i repeat this revolve. Any clue why it happens and how i solve it ?

Unfortunately i overwrote the file i had saved with that error but i'll redo it again and post the file here once i have time to do it.
