I've updated the original post with a screenshot and ZIP file for the MoI beta 4.0 (Oct. 17, 2018) release.

There were a few tricky things to figure out, but I believe the user interface should be good to go. I tested it briefly on Windows 8. It also features more new icons than before and an updated Inkscape SVG file.
