Blank in this context just means "no objects".
So set up your styles as you want, but do not create any objects - then save out as a 3DM file. The file will contain no objects but it will contain a style list still.
When you set it as a template, that file will be loaded as the starting environment when you start up MoI or pick File / New.
You can have objects in there as well, if you always want some particular objects to be included in your environment when you start up MoI each time, but usually most people don't want there to be any objects in their model when they initially start the program, hence the "blank" part.
